Description - Description: The Ontario Palliative Care Network is seeking expressions of interest from palliative care health service providers for the position of Clinical Lead, Models of Palliative Care: Hospital for hospital settings. This work will leverage th e successful development of the Health Services Delivery Framework for adults in the community.This is a unique opportunity to provide strategic leadership for clinical system improvements by leading the knowledge translation and exchange efforts required to build awareness and support change management for the recently developed palliative care model of care for adults in hospital settings in Ontario.
