Description - Description: The competition concerns the delivery of a solution for digital home follow-up (DHO), with the intention of enabling holistic and coherent DHO services for patients/users who receive follow-up, also across service levels. The solution, i ncluding measuring equipment, is delivered as ongoing services. In the Health Partnership Ahus and the municipalities, there is an ongoing joint collaboration project within digital home monitoring in which Ahus and 10 of the municipalities are included. In this project, digital home follow-up is organized in close cooperation between the municipal follow-up service, specialist and GP, which promotes the quality of the digital self-treatment plan
