Description - Description: The aim of this service is to deliver person centred, holistic support for people in their own home for the duration of the contract. LBTH is working in partnership with a Registered Provider to supply units for this project, with additi onal units being provided from the council’s own housing stock. These homes will all be located within LBTH.The service is for an initial three year period with two 1 year extensions possible The aim of this project to provide housing outcomes for some of the most multiply disadvantaged and excluded LBTH residents. Specifically, this is a Housing First project which will provide homes and intensive wrap-around support to people rough sleeping.
