Description - Description: The Ministry of Health (Ministry) is seeking proposals to develop and perform an evaluation of the Long-Term Care at Home (LTC@Home) Program.Program monitoring and evaluation is needed to measure program outcomes, identify opportunities for improvement, and inform provincial program expansion planning.
