Description - Description: Northern Queensland Primary Health Network (NQPHN) invites applications from service providers interested in supplying a multidisciplinary allied health team using a ’work-in, work-out' model of care for underserviced communities withi n the NQPHN western corridor. This tender covers rural and remote areas of the Townsville Hospital and Health Service (HHS) region. Funded services will be delivered through block-funding and focus on the care of chronic conditions in alignment with the Chronic Conditions Management Plans developed by local general practitioners.
