Description - Description: Provision of an ED (Emergency Dept) Primary Care Streaming service on behalf of University Hospitals Birmingham NHS Foundation Trust (UHB) under the Provider Selection Regime (PSR) Competitive Process. A significant proportion of self-at tending patients present to the Emergency Departments (ED) at UHB with conditions that are able to manage within primary care. Whilst measures are in place to re-direct many of these patients back into primary care are in action, there is always likely to be the need for provision of primary care services.The successful provider will:• see patients after initial nurse navigation into the primary care stream, working alongside the ED team to see
