INTRODUCTION & BACKGROUND
LEHS, a not-for-profit organization works towards improving the quality and access to primary healthcare services for underserved communities. WISH (Wadhwani Initiative for Sustainable Healthcare) is a flagship program of LEHS in India. WISH seeks to improve the Indian primary healthcare system through innovation, and make quality healthcare available, accessible, and affordable for all. The Organization is working on primary healthcare models through a partnership with State governments, international agencies, and the private sector by appropriately introducing promising healthcare innovations in the public health system in order to build an equitable, innovation-led, and evidence-based quality healthcare system within easy reach of the underserved populations of Rajasthan, Madhya Pradesh, Uttar Pradesh, Meghalaya and Assam.
PURPOSE
These bags will be used by ASHA’s for carrying their daily logistic items needed during their field and household visits. These bags will be use by ASHA’s for urban and rural area of the Bhopal district
TIMELINE
All bags should be received in Bhopal office address as mentioned above within 15 days after receiving the PO.
REQUIRED QUALIFICATION OF THE AGENCY
Expertise in Manufacturing or supply of the bags
Should be registered as similar type of work
Self-declaration certificate for last three working

PROPOSAL SUBMISSION DEADLINE
All proposals must be submitted by 11 th March 2024, with subject line "RFP for supply of bags for ASHA workers in Bhopal district- MRF195
Proposal Submission Guideline
All interested vendors must submit their proposals in accordance with the guidelines outlined below;
Vendor are expected to submit the proposal with the detail and specifications along with cost proposal pertaining to the quantity mentioned as per RFP.
Proposals received after the deadline will not be considered

Please submit the following as enclosures or attachment with your quotation. Bidders must provide all the information requested below. Quotations that do not provide the required information and certificates, or do not follow the submission requirements may not be reviewed.
Company Profile, testimonials, work completion reports, purchase orders, reference (NGOs).
GST registration certificate.
PAN registration.
TAN registration.
Audited Financials for last three years (3 pages).
Income Tax Return for last three years.
Cancelled cheque.
Copy of registration documents/certificate and most recent renewal as a legal entity.
Preferably Private Ltd company and has sound business records.
All responses to this RFP must be received no later than 11th March 2024. The proposal should be submitted only through e-mail in PDF format addressed to CEO, LEHS|WISH Foundation on the below-given e-mail id: rfp@wishfoundationindia.org
All inquiries and requests for information affecting this assignment must be submitted no later than 7th March 2024, 4:00 PM by e-mail at the above e-mail id. Inquires and answers to inquires will be shared with all. WISH will not compensate agencies for its preparation of response to this RFP.
Only shortlisted organizations will receive an acknowledgment and will be called for personal interactions-presentation / negotiations. Organizations that do not hear from us within two weeks from the last date of submission of the proposal may assume that their application has not been successful.
LEHS|WISH reserves the right to reject any or all proposals and to negotiate terms and conditions with the selected vendor. The acceptance of any proposal is at the sole discretion of LEHS|WISH.
