| Description | Memo No: JGMCH/2O23/54V QUATATION NOTICE Date: Sealed quotation are invited from the reputed Company/Direct Importers/Authorized distributor firms for supply of the following items as mentioned below for the use of Jhargram Govt. Medical College & Hospital. The last date of submission of quotation is 1^*03 *2.3 on_2.00_ PM and the same will be opened on the same date at_3.00_ PM in presence of interested bidder, if any. The bidder will submit the quotation in their own official pad along with the following essential documents. 1) Valid Trade License 2) PAN card of the Bidder/Bidder Company. 3) Manufacturing License (in case of manufacturer) 4) Latest GST return 5) Latest IT return 6) Up to date P.Tax 7) Valid Drug license 8) The rate will have to quote both in word and figures excluding GST in their own respective letter heads of concern agencies. 9) It is strictly directed to selected bidder to supply the following item within 10 days after order placed. Approval of purchase committee for this Quotation Notice has already been taken. The quotation is to be submitted in the drop box which will be kept at the office chamber of the undersigned on all working days from ■ 2.3 to up to 2.00_ PM. Physical verification of samples and/or Physical demonstration may be done at the discretion of the hospital authority by a team of expert to adjudge the applicability or suitability of the product or products in the functional requirement of the patient since the items are life savings and the technical evaluation will be done accordingly in close observation of the product or products, technical brochures along with the Test Certificate produced and physical sampling thereof.
